Summary of Duties

Provides quality care to patients in their homes under the direction of a physician.  The nurse evaluates safety concerns and coordinates home health care services that reflect the home health care agency philosophy and standards.

Hiring Competencies

a)    Education: Bachelor’s degree in Public Health or Nursing preferred. A copy of your highest academic diploma or transcript required upon hire.

b)    License: Current license to practice professional nursing in the state of Oregon required. 

Current Oregon or Washington Drivers license and at least 4 years of holding a driver’s license.

No more than 1 accident or moving violation on or off the job within the past 12 months and no conviction for driving under the influence of intoxicants (DUII) within the past 5 years.

Current automobile accident insurance with coverage limits of $100,000/$300,000/$100,000 or combined $300,000 limits and name Tuality Healthcare as the “additional insured”.

c)    Certification:  Current Healthcare Provider BLS required or within 90-day of hire.

d)    One-year experience in hospital medical/surgical nursing preferred.

e)    One-year experience in home health care or public health nursing preferred.

f)     Experience in physical assessments of geriatric patients preferred.

g)    Critical care, oncology, or intravenous skills preferred.

h)    Excellent customer service skills.

i)     Ability to work without direct supervision required.

j)     Excellent communication skills.

k)    Knowledge of nursing principles and practices required.

l)     Knowledge of the nursing process and its applications to patient care required.

m)   Provides care appropriate to the age of patients served on the job.

n)    Must be able to develop skills in use of computerized information/Allscripts system used in home care setting.

o)    Excellent documentation skills required.

p)    Professional personal appearance makes a good first impression.
