Provides support services in patient's home which are required to maintain normal bodily function and emotional comfort and to assist the patient toward independent living.
a) Education: Completion of Nurse Aide training program approved by
the Oregon State Board of Nursing. Meets
the standards of competency for Home Health Aide.
b) License: Oregon Home Health Aide certificate preferred. Certified Nursing
Assistant certificate required. Current
Healthcare Provider BLS required or within 90-days of hire.
c) Current Oregon Drivers license and at least 4 years of holding a driver’s
d) No more than 1 accident or moving violation on or off the job within the past
12 months and no conviction for
driving under the influence of intoxicants (DUII) within the past 5 years.
e) Provides care appropriate to the age and functional status of patients served
on the job.
f) Follows oral and written instructions accurately and completely.
g) Demonstrates behaviors that show a high regard and worth of patients and
h) Uses effective communication skills
i) Communicates willingness to work cooperatively as a team member.
j) Personal appearance makes a good first impression.
