Summary of Duties

Responsible for providing and coordinating pharmaceutical care services and collaborative drug therapy management for patients.  In addition, will serve as a resource to the pharmacy, nursing and medical staff within the field of anticoagulation.  

Hiring Competencies

a)      Education: Graduate of an accredited college of pharmacy. Pharm.D. or M.S. with

          residency or equivalent work   experience preferred

b)      License: Current valid pharmacy license by the Oregon Board of Pharmacy.

c)      Certification: Certified Anticoagulation Provider preferred or working toward certification.

d)      Experience: Experience with Coagucheck PT Testing and Anitcoagulation clinic

          experience preferred.

          Minimum of two years of experience in hospital or ambulatory care pharmacy preferred.

e)      Excellent customer service skills.

f)       Comprehensive clinical pharmacy knowledge base with emphasis in the field of anticoagulation.

g)      Knowledge of and understands coagulation and thrombogenesis

h)      Knowledge of and understands management of drug-drug and drug-food interactions associated with


i)       Knowledge of and understands the pharmacokinetic and pharmacodynamic properties of warfarin

j)       Knowledge of and understands the disease processes and medical therapies affecting anticoagulation.

k)      Knowledge of and understands the current guidelines for managing anticoagulation, associated risks, and

           patient assessment

l)       Presents self in a professional manner.

m)     Demonstrated ability to communicate effectively in verbal and written form with the health care team and

          with patients.

n)      Excellent interpersonal communication skills.

o)        Bilingual skills a plus.
