    This position is responsible for ensuring the delivery of quality nursing care to the community and clients of ¡Salud! Services.  The nurse will follow the established ¡Salud! Services care standards and protocols under the supervision of the ¡Salud! Clinical Services Manager (CNM).  This position is responsible for assisting the ¡Salud! Program Team in building and maintaining relationships between patients, community, providers, organizations, and vineyard owners.  Duties will require competency in nursing assessment, patient education, program development and education.

    Hiring Competencies

    a)   Education: Experience in community health education preferred. A copy of your highest academic diploma or transcript required upon hire.

    b)   License: Current Oregon State RN license

    c)   Certifications:

                         1. American Heart Association Healthcare Provider BLS required upon hire.

                         2. Become Medic First Aid CPR instructor within 180 days from required.

                         3. Obtain Living Well With Chronic Conditions /Tomando Control de su Salud certification within 180 days of hire

    d)   Drives a Tuality owned vehicle as part of their job and is considered a Category A driver.

                         1. Must have a valid Oregon drivers license for a least 4 years and be at least 21 years old with minimum of two years’ experience with the type of vehicle being driven.

                         2. Must have a satisfactory driving record which is defined as no more than one accident or moving violation on or off the job within the past 36 months, and no conviction for driving under the influence of intoxicants (DUII), license suspension, auto felony convictions or other serious violations within the past 36 months.

                         3. This employee must complete a defensive driving course within the first 90 days of employment and annually thereafter.

    e)   Bilingual, bicultural in Spanish required; able to present and provide health literature to all age groups.

    f)    Conducts nursing assessment of clients including physical examinations and/or interviews to obtain and document pertinent physical and social and or educational background to detect or identified problems.

    g)   Provides intermittent skilled nursing services such as, blood pressures, vaccinations, finger sticks, teaching and evaluation.

    h)   Develops follow up plans for clients requiring continuing assistance, serves as a referral agent and advocate.

    i)    Coordinates client care activities and maintain communication with team members and other disciplines. 

    j)    Uses effective communication skills with customers of diverse ethnic and cultural backgrounds.

    k)   Uses effective problem solving skills and works well independently.

    l)    Knowledge of Medical insurance procedures and workers’ compensation procedures.

    m)  Makes home visits to clients to assess and assist with requests made.

    n)   Demonstrates proficiency with a PC using Word, Excel, Power Point, Access and Lotus Notes.

    o)   Flexibility in work schedule, ability to work weekends, evening programs and travel throughout the Willamette Valley.

    p)   Assist the Clinical Nurse Manager in the assessment of ¡Salud! Clients and community health educational needs.

    q)   Works with the Clinical Nurse Manager, the Health Educator and Community Health Department Coordinator and staff in developing educational programs in health related areas.  Promotes wellness and prevention.

    r)    Demonstrates effective organizational skills and ability to maintain accurate and timely records and reports.

    s)   Demonstrates excellent customer service and telephone skills.

    t)    Demonstrates proper appearance and personal conduct for the employee's particular job.
