    Provides quality care to patients in their homes under the direction of a physician.  The nurse evaluates safety concerns and coordinates home health care services that reflect the home health care agency philosophy and standards.


    a.   Education: Bachelor’s degree in Public Health or Nursing preferred. A copy of your highest academic diploma or transcript required upon hire.

    b.   License: Current license to practice professional nursing in the state of Oregon required. 

    c.   Certification: American Heart Association Healthcare Provider BLS required upon hire.  

    d.   Current Oregon or Washington driver’s license, be at least 21 years old with minimum of two years’ experience in the type of vehicle being driven. No more than one accident and two moving violations or two accidents and one moving violation or no accidents and three moving violations on or off the job within the past 36 months and no conviction for driving under the influence of intoxicants (DUII), license suspension, auto felony conviction or other serious violations within the past 36 months. 

    e.   Clearance of a criminal background check and sex offender check is required every 3 years of employment as required by OAR 333-027-0064.

    f.    One-year experience in hospital medical/surgical nursing preferred.

    g.   One-year experience in home health care or public health nursing preferred.

    h.   Experience in physical assessments of geriatric patients preferred.

    i.    Critical care, oncology, or intravenous skills preferred.

    j.    Excellent customer service skills.

    k.   Ability to work without direct supervision required.

    l.    Excellent communication skills.

    m.  Knowledge of nursing principles and practices required.

    n.   Knowledge of the nursing process and its applications to patient care required.

    o.   Provides care appropriate to the age of patients served on the job.

    p.   Must be able to develop skills in use of computerized information/Allscripts system used in home care setting.

    q.   Excellent documentation skills required.

    r.    Professional personal appearance makes a good first impression.

    s.   Bilingual skills a plus.
