    Provides occupational therapy assessment, treatment and instructions to patients under the direction of a physician.  Collaborates with the case manager to provide home health services that reflect the home health agency philosophy and standards.

    Hiring Competencies

    a)    License: Current Oregon license in Occupational Therapy required. American Heart Association Healthcare Provider BLS preferred upon hire, and required within 90 days of hire. If not American Heart Association Healthcare Provider BLS certified upon hire, must have current BLS certification through another provider. ACLS certification required within 6 months of hire.

    b)    Current Oregon Driver’s license and be at least 21 years old and have a minimum of two years’ experience in the type of vehicle being driven.  No more than one accident and two moving violations or two accidents and one moving violation or three moving violations and no accidents on or off the job within the past 36 months.  No conviction for driving under the influence of intoxicants (DUII), license suspension, auto felony convictions or other serious violations within the past 36 months.

    c)    Clearance of a criminal background check and sex offender check is required every 3 years of employment as required by OAR 333-027-0064.

    d)    One year experience in home health or other health setting with geriatric emphasis preferred.

    e)    Provides care appropriate to the age of patients served in the job.

    f)     Knowledge of occupational therapy assessment and treatment methods required.

    g)    Utilizes occupational therapy modalities and equipment.

    h)    Maintains effective relations with other health care professionals.

    i)     Uses effective communication skills.

    j)     Personal appearance makes a good first impression.

    k)    Bilingual skills a plus.
