  • Summary of Duties

    To provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review and discharge planning. Has accountability for the care, coordination and discharge planning of all patients being treated in the Emergency Department .

    Hiring Competencies

    1. Education: Completion of course(s) in Case Management preferred.
    2. License: Current license as a Registered Nurse by the Oregon State Board of Nursing.
    3. Certification: American Heart Association Healthcare Provider BLS required upon hire.
    4. Minimum of two (2) years of utilization review/case management experience
    5. Minimum of one (1) year experience in discharge planning from a hospital is preferred.Strong clinical nursing skills with at least 5 years hospital experience in varied clinical settings.
    6. Demonstrates ethical commitment to quality assurance and confidentiality of all data and information.
    7. Emulates THC dress code and Code of Conduct that incorporates presenting a good first and lasting impression by personal appearance and interpersonal skills.
    8. Advanced communication and interpersonal skills that utilizes consideration body language, filters, listening, paraphrasing, and questions with customers of diverse ethnic and cultural backgrounds.  This also includes the skills to obtain and interpret information appropriate to patients' needs and age as required for assessment, range of treatment and patient care.
    9. Knowledge of community health and social service resources preferred.
    10. Ability to write clear and legible clinical notes.
    11. Experience working in highly confidential settings.
    12. Possesses excellent telephone skills and interpersonal communication abilities both verbal and written.
    13. Ability to develop case management care plans and solutions utilizing problem-solving skills.
    14. Familiar with computerized word processing, database and spreadsheet programs.
    15. Ability to implement ideas and programs required.
    16. Self directed and must be able to follow through on projects with attention to detail and implement ideas and programs with minimal supervision.
    17. Demands creativity, flexibility and the ability to work under stress.
    18. Bilingual skills a plus.
