We are recruiting for two positions in two locations listed below.
1. Primary Care - North Plains
2. Primary Care - Forest Grove
The Nurse Practitioner will provide health care services for a broad range of patients; including geriatric, adults, adolescents and infants. Service includes but is not limited to preventative medicine, chronic disease management, acute illness and procedures, chronic pain, and behavioral health management. Services will be reviewed and approved by the Medical Director through periodic chart review.
KEY RESPONSIBILITIES – |
Performed majority of the time -- |
· Maintain regularly updated patient medical records that document all history, evaluations, medications, treatments, allergies, test results and other records · Performs physical exams · Administers medications via all routes safely and accurately according to procedure. · Provides professional service to patient in the Primary Care Clinic. · Delegates and coordinates tasks and duties of the healthcare team. · Renders all services promptly and diligently in accordance with professional and ethical standards of the medical community in which the nurse practitioner practices.

· Adapts care to accommodate unexpected changes in patient status. |

JOB SPECIFICATIONS | ||
Education: | Required | · Completion of a recognized Nurse Practitioner program. · A copy of your highest academic diploma or transcript. |

Preferred | · Minimum of one to three (1-3) years’ experience as a Nurse Practitioner. | |
Licenses, Certifications and/or Registrations: | Required | · Licensed to practice medicine as a Nurse Practitioner by the State of Oregon/Oregon State Board of Nursing. · American Heart Association Healthcare Provider BLS. · Fulfills requirements of hospital medical staff credentialing process. · Maintains active staff membership in good standing with the medical staff of the hospital with all privileges and responsibilities accorded such membership. |
