POSITION SUMMARY

We are recruiting for two positions in two locations listed below.

 

1. Primary Care - North Plains 

2. Primary Care - Forest Grove

 

The Nurse Practitioner will provide health care services for a broad range of patients; including geriatric, adults, adolescents and infants. Service includes but is not limited to preventative medicine, chronic disease management, acute illness and procedures, chronic pain, and behavioral health management. Services will be reviewed and approved by the Medical Director through periodic chart review.

 

KEY RESPONSIBILITIES – 

Performed majority of the time -- 

 

· Maintain regularly updated patient medical records that document all history, evaluations, medications, treatments, allergies, test results and other records

· Performs physical exams
· Evaluate a patient’s symptoms and determine the most appropriate treatment plan to address them
· Prescribe medications in compliance with state and federal statutes and inform patients of all possible risks, complications and interference with other medications they may be taking
· Consult patients on topics such as proper nutrition, fitness and hygiene to assist them in making informed decisions and taking good care of their health
· Perform minor procedures
· Make appropriate referrals
· Offers counseling and education to meet patient needs

·  Administers medications via all routes safely and accurately according to procedure.

·  Provides professional service to patient in the Primary Care Clinic.

·  Delegates and coordinates tasks and duties of the healthcare team.

·  Renders all services promptly and diligently in accordance with professional and ethical standards of the medical community in which the nurse practitioner practices.

 

Performed occasionally but critical to successful performance of the job:

·       Adapts care to accommodate unexpected changes in patient status.

Decision making and budget responsibilities (e.g. impact limited to the employee, entire unit or organization-wide):

·       Impact limited to the employee.

Education:

Required

·       Completion of a recognized Nurse Practitioner program. 

·       A copy of your highest academic diploma or transcript.

Experience:

·       Minimum of one to three (1-3) years’ experience as a Nurse Practitioner.

Licenses, Certifications and/or Registrations:

Required

·       Licensed to practice medicine as a Nurse Practitioner by the State of Oregon/Oregon State Board of Nursing.

·       American Heart Association Healthcare Provider BLS.

·       Fulfills requirements of hospital medical staff credentialing process.

·       Maintains active staff membership in good standing with the medical staff of the hospital with all privileges and responsibilities accorded such membership.
