Provides comprehensive medical care to all patients served. Performs all duties in a manner, which promotes a team concept and reflects the mission and vision. KEY RESPONSIBILITIES – Performed majority of the time -- · Physician shall cooperate in the establishment of procedures and methods of operation for the office, maintaining in good standing medical staff privileges including attendance at required meetings.· Performs and document assessments and data collection in an ongoing, timely, accurate, concise and systematic manner.· Physician shall render professional services in the office designated and provided by the hospital in accordance with the Physician Employment Agreement. · Provides inpatient care for the patients of the office practice and related referral patients. · Renders all services promptly and diligently in accordance with the professional and ethical standards of the medical community in which the physician practices. Performed occasionally but critical to successful performance of the job: · Adapts care to accommodate unexpected changes in patient status.

Position Summary – The Nurse Practitioner will perform medical services to the Wound Care clinic patients in accordance with individual needs and the standards of care as represented by the centers patient care policies. Services will be reviewed and approved by the Medical Director through periodic chart review. KEY RESPONSIBILITIES Performed majority of the time -- : · Performs and documents patient assessments and intervention in a timely, accurate, and concise manner, utilizing departmental charting formats including electronic medical records systems.· Administers medications via all routes safely and accurately according to procedure.· Assists with emergency treatments, following departmental policies and procedures.· Provides professional service to patients in the Wound Care clinic services.· Delegates and coordinates tasks and duties of the healthcare team. Performed occasionally but critical to successful performance of the job: · Adapts care to accommodate unexpected changes in patient status. We are recruiting for two positions in two locations listed below. 1.Primary Care - North Plains 2. Primary Care - Forest Grove The Nurse Practitioner will provide health care services for a broad range of patients; including geriatric, adults, adolescents and infants. Service includes but is not limited to preventative medicine, chronic disease management, acute illness and procedures, chronic pain, and behavioral health management. Services will be reviewed and approved by the Medical Director through periodic chart review. KEY RESPONSIBILITIES – Performed majority of the time -- · Maintain regularly updated patient medical records that document all history, evaluations, medications, treatments, allergies, test results and other records· Performs physical exams· Evaluate a patient’s symptoms and determine the most appropriate treatment plan to address them· Prescribe medications in compliance with state and federal statutes and inform patients of all possible risks, complications and interference with other medications they may be taking· Consult patients on topics such as proper nutrition, fitness and hygiene to assist them in making informed decisions and taking good care of their health· Perform minor procedures· Make appropriate referrals· Offers counseling and education to meet patient needs· Administers medications via all routes safely and accurately according to procedure.· Provides professional service to patient in the Primary Care Clinic.· Delegates and coordinates tasks and duties of the healthcare team.· Renders all services promptly and diligently in accordance with professional and ethical standards of the medical community in which the nurse practitioner practices.
